Residence permits for EU/EFTA/Swiss nationals
A national of the European Union staying in the Republic of Poland for a period exceeding 3 months has the obligation to register his/her stay
Required documents (in Polish or translated into Polish by a sworn translator):

2.    photocopy of a valid travel document (the page with personal details e.g  passport, ID card, driver license)  or another document confirming his/her identity and citizenship,
3.    certificate from a public college confirming admission to a college or referral to a professional training course,
4.    a document confirming medical insurance coverage or rights to avail of healthcare services pursuant to regulations on coordination of social security systems
5.    a written declaration that the person has sufficient financial resources to provide for himself/herself and his/her family members, without the necessity to avail of social assistance services, or a certificate confirming that the person has such financial resources.

The documents should be submitted in person in the Foreigners' Affairs Unit of the Department of Nationals' and Foreigners' Affairs of Małopolska Provincial Office (in Polish Małopolski Urzęd Wojewódzki)  in Krakow, ul. Przy Rondzie 6 (ground floor, foreigners' affairs room).

Other countries

Students from outside EU/EEA must apply  national student visa (D10) issued for up to one year.

a/ general rules:
1.    application with other  documentation must be submitted in person at the consul of Republic of Poland  in the respective country;
2.    All documents must be written in Polish or foreign language indicated by the consul;
3.    visa procedure takes up to 15 calendar.


b/  required documents:
 
1.    complete application form for the issue of a national visa;
2.    passport (for inspection) + 1photocopy , with a validity of at least 3 months from the date of completion of the planned stay in Poland, containing at least two free pages and issued in the past 10 years;
3.    1 photo; must be in color, with dimensions of 35 mm x 45 mm, taken in the last six months on a plain white background ; photograph must show the person without a hat and dark glasses, looking straight ahead with open eyes, not covered by hair , the natural expression of the face and mouth closed;
4.    documents proving the fulfillment of the conditions necessary for adoption at the Polish university (e.g. document from Polish universities approved certifying the qualifications of the candidate to undertake studies in Poland in accordance with the Polish law)
5.    documents confirming sufficient financial resources or the ability to obtain funds to cover the cost of living during the stay in Poland and the return to the country of residence ( scholarship);
6.    health insurance or travel medical insurance in the minimum amount of insurance in the amount of EUR 30,000.
Note: Consul at every stage of the visa may request supporting documents.
